Create a Form Yourself

From a blank form to a live one, in order

Building a form by hand takes you through a short sequence of screens, each one handling a single decision. Zyntro walks you through them in order the first time. After that, every screen is reachable directly from the My Forms list so you can change one thing without repeating the rest.

This guide follows the first run all the way through.

Before You Begin

Steps

Open My Forms and start a new form

Go to Sales → CRM → Forms. Click Make a New Form at the top right, or Create a New Form if you have no forms yet.

You will be offered two ways to build. Choose Design Manually to build the form yourself.

Open My Forms and start a new form
Build Me a Form is the AI route. It is covered in its own guide.

Name the form

The Form Name is for you, not for the people filling it in. It is what you will see in the My Forms list and in reporting, and it becomes the contact's source — so every contact created by this form will show this name as where they came from.

Name it after what it does: Website Enquiry, Demo Request, Client Onboarding. Avoid version numbers and dates; you can duplicate and rename later.

Decide whether the form is public

This is a public form is on by default. Leave it on.

Turning it off reveals a password and confirmation field. Set that expectation carefully — see the note below this step.

Attach a schedule if the form should take bookings

Add a Schedule for Bookings? is on by default, so check it before you continue. If you leave it on, pick a schedule from the list underneath.

Attaching a schedule adds a calendar step to the form. The person completing it picks a real available time, and on submission Zyntro creates the contact, the calendar event and the meeting together, then emails both sides an invitation.

If this form is not for booking anything, switch the toggle off. You can attach or remove a schedule later from inside the form builder.

The booking toggle starts on. A form that only collects enquiries does not need it.

Click Continue and choose about the welcome screen

Zyntro creates the form and asks whether you want a welcome screen — a full-screen introduction shown before the first question.

Give me a Welcome Screen takes you to the designer. Just the Form please! goes straight to the field builder.

A welcome screen suits a form you send people to as a destination. Skip it for a short form embedded in a page, where an extra click costs you completions.

Add your fields

You are now in the form builder. The left panel lists every field type grouped into categories; the middle is your form; the right is the settings for whichever field you have selected.

Drag a field onto the canvas, or click it. Then select it and work through its settings on the right — its label, whether it is required, its validation, and Map Data To, which decides where the answer ends up.

Click the form title at the top of the canvas to rename the form inline.

Add your fields
Set Map Data To as you add each field. It is much slower to revisit twenty fields afterwards.

Preview, then continue

Preview Form opens the form exactly as a visitor sees it, and lets you check it at desktop, tablet and phone widths. Save Progress saves without leaving.

When the fields are right, click Continue to move on to styling.

Style the form

Set your colours, input sizes and shapes, button styling and heading sizes. A live preview beside the controls updates as you change things.

See the styling guide for what each control affects.

Set up what happens on submission

Saving your styles takes you to Form Actions, the last screen in the sequence. This is where you choose the thank-you message or redirect, any follow-up email or SMS, whether to trigger a call, who owns the lead and who gets notified.

You can accept the defaults for now and come back — but you do need to save this screen, because saving it is what makes the form live.

Save Form Actions to publish the form

Saving Form Actions for the first time changes the form's status from Draft to Published and returns you to My Forms.

There is no separate Publish button. Publishing is a side effect of saving this screen.

Published means Segmentation Intelligence can now see and use this form.

Important: The password option is not enforced yet, as of October 2026. Turning off This is a public form and setting a password stores the password, but nothing currently checks it — the form stays reachable at its public address. Treat every form you build as publicly accessible, and do not rely on the password to protect sensitive questions.

Warning: Draft does not mean private. A Draft form is already reachable at its public address — anyone with the link can complete it. What Draft actually controls is visibility to Segmentation Intelligence: while a form is Draft, SI cannot see it, cannot reference it in email, SMS or broadcasts, and it will not appear to connected AI agents. If you need a form genuinely unreachable while you work on it, build it and share the link only when you are ready.

Result

Your form appears in My Forms marked Published. Its name in that list links to its public address, which is where you send people.

From the row menu you can reopen any screen individually — Edit Fields, Edit Welcome Screen, Manage Styles, Manage Form Actions — plus View Completions to see submissions, Duplicate to clone the form, QR Code to generate a printable code, and API Payload to see the shape of the data a submission produces.

After a form is published, the screens no longer chain together. Saving your styles returns you to the list rather than pushing you into Form Actions.
